These Terms & Conditions (“Terms”) govern the use of services provided by Cog Eleven (“we”, “our”, “us”). By engaging our medical or medicolegal services, you agree to be bound by these Terms. Please read them carefully.
2. Scope of Services
We provide:

Private medical consultations (clinical assessments, reports, treatment advice), and

Medicolegal services (independent expert reports, case reviews, and court attendance).
We do not provide emergency medical care. In urgent or life-threatening situations, you must contact emergency services (999 in the UK) or attend your nearest hospital.
3. Appointments & Cancellations

Appointments must be booked in advance.

Cancellations made with less than 48 hours’ notice may be charged in full.

We reserve the right to cancel or reschedule appointments due to unforeseen circumstances (e.g., illness, court commitments).
4. Fees & Payment

Fees are confirmed prior to appointment or engagement.

For medicolegal services, fees may be agreed under a written retainer or service agreement.

Interest may be charged on late payments in accordance with statutory rights.
5. Confidentiality & Data Protection

All medical and legal information is treated with strict confidentiality.

We comply with data protection laws, including the UK GDPR and Data Protection Act 2018.

6. Medicolegal Independence
Where we provide expert witness services, we have a duty to the Court and must remain independent and impartial, regardless of which party instructs us.
7. Limitation of Liability

Our services are provided with reasonable skill and care in accordance with professional standards.

Losses caused by misuse of our reports or advice,

Consequences of withholding or providing inaccurate information,

Any indirect, incidental, or consequential loss.

Nothing in these Terms excludes liability for death or personal injury caused by negligence, fraud, or any other liability that cannot be excluded under law.
